Back Injury : Chronic Back Pain and Strain
This refers to back pain, upper or lower, that has been a continual problem for months to years. This type of back problem is usually secondary to inflammation that has developed from an old injury to the muscles or ligaments.
Any tearing injury to the muscle is referred to as a strain. A similar injury to the ligaments [which support the bones of the spinal column] is known as a sprain. Both of these injuries cause similar pain. Normally, these injuries heal within several weeks with no lasting pain (see acute back strain or sprain). Some patients develop a more chronic problem. Typically, exercise, or even minor physical activity, can aggravate the back pain.
SELF HELP MEASURES
* Aspirin or anti-inflammatories reduce chronic pain.
* Bedrest on a firm mattress, in a position of comfort.
* Moist warm compresses to the back can reduce muscle spasm (do not use heat on acute injuries in the first 24 hours).
* Avoidance of activities which aggravate symptoms. In some people this can mean a career change.
* Good posture while sitting, or a lumbar support pillow for extended sitting.
* Women should avoid high-heeled shoes.
* Special care when lifting objects - keeping the back straight and bending the knees.
* Muscle strengthening exercises - increasing abdominal muscle tone and strength (leg lifts, modified sit-ups).
* Weight reduction where appropriate.
An Orthopaedic Surgeon can be helpful in the management of this problem. The physician can also arrange physical therapy that will help you eliminate the problem. Further evaluation may be necessary to exclude the possibility of disc disease.
